Are you an experienced LVN with a passion for home health care? We are seeking a dedicated Office Manager / Care Coordinator to join our dynamic team! This is an exciting opportunity to play a pivotal role in our rebranding journey and help us set new standards in patient care.

Key Responsibilities :

  • Home Health Expertise : Utilize your experience to manage the entire referral process from the initial referral through to the final billing. Ensuring smooth operation and high-quality care throughout the process. You will use your knowledge to approve the F2F and the DX code given by the MD. This candidate will be responsible for submitting the NOA, calling for insurance auth or obtaining Medicare PCR number.
  • Documentation : Complete all necessary documentation accurately and timely, in accordance with company policies.
  • Team Collaboration : Work closely with our care team, physicians, referral sources, family members, and patients to coordinate care effectively.
  • Candidate should be comfortable with using an electronic clinical record. Able to run the necessary reports, use email and have basic computer skills.
  • Field Support : Assist field nurses with follow-up on physician orders and collaborate on patient care needs. Also, maintain supply closet with equipments / supplies.
  • Leadership Partnership : Collaborate with leadership to develop and implement new processes that improve our services. This person will play a critical role in the leadership team.
  • Participate in the HR process : including new hires and on boarding for new staff, payroll
  • Must be available to perform pt visits if needed and participate in On-Call Rotation.
  • Must be willing to participate in marketing / sales events. Making visits to physician offices or ALF's.
