Key Responsibilities :
- Provide comprehensive care for patients of all ages, managing chronic and acute conditions.
- Collaborate with specialists, referring physicians, and other healthcare professionals to ensure coordinated care for patients.
- Stay current with advancements in medical practice and integrate evidence-based care into daily practice.
- Document patient interactions and treatment plans in accordance with system protocols.
- Participate in quality improvement initiatives and contribute to a patient-focused care model.
