A company is looking for a Pediatric / NICU Case Manager, RN - Remote Living in WA.\n\nKey Responsibilities\n\nEngage members to complete comprehensive needs assessments addressing medical, behavioral, and socioeconomic needs\n\nDevelop and implement person-centered care plans in alignment with evidence-based guidelines\n\nCollaborate with care teams and community resources to support member self-management and coordinate care transitions\n\nRequired Qualifications\n\nCurrent, unrestricted RN licensure in the state of Washington\n\n2+ years of NICU and / or Pediatric clinical experience\n\n1+ year of experience with MS Office applications\n\nReside in the state of Washington\n\nDesignated workspace with secure high-speed internet access
