Your Role as a Home Health Physical Therapist

Here’s a snapshot of what you’ll be doing :

  • Deliver home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury
  • Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care and goals
  • Observe movements, conduct assessments, document progress, and adjust care plans as necessary
  • Guide patients through exercises and hands-on therapy to alleviate pain and enhance mobility
  • Educate patients about their care plans, goals, exercises, and self-care techniques
  • Support families in understanding the care plan and safely assisting with exercises

    • What We’re Looking For

    A few must-haves for our Home Health Physical Therapists :

  • Graduation from an accredited Physical Therapist Program and an active PT license in Michigan
  • A minimum of 1 year of physical therapy experience, preferably in home healthcare within the last 5 years
  • Current CPR certification
  • Familiarity with state and federal home health regulations
  • Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach
