RN Post-Acute Care Manager

A company is looking for a Post-Acute Care Manager - Population Health.

Key Responsibilities

Run daily reports for transitions of care and review goals of care

Communicate regularly with skilled nursing facilities to encourage progression of care

Conduct comprehensive assessments and coordinate referrals and transitions of care

Required Qualifications, Training, and Education

Current RN License in the state of practice or covered by compact

Bachelor of Science in Nursing (BSN) preferred

2 years of nursing experience required

1 year of experience in skilled nursing settings preferred

EPIC Electronic Health Record training preferred
