Psychiatric Rehabilitation Services Coordinator

A Skilled Nursing Facility is seeking a Psychiatric Rehabilitation Service Coordinator (PRSC) to whom the resident primarily relates for the coordination of services, programs, oversight of data collection, and review of resident performance.

The candidate must meet the following qualifications :

• Minimum of one year working with the adult mentally ill population; preferred but not required.
• BA Degree in Human Services field (including sociology, social work, special education, and psychology).
• Experience with caseloads, crisis intervention, documentation, care planning involving families in the process, knowledge of the MDS, discharge planning, facilitating skills groups, psycho-social groups, and problem-solving.
• Good assessment skills, interpersonal skills, and knowledge of MDS 3.0 along with basic computer skills, communication, advocacy skills, and charting experience.
• Must be able to work in an interdisciplinary team environment.
• Must be able to handle confidential data with professional discretion.
• Position is 5 days / week, 8 hours per day.
