About the Role

We are seeking a motivated and experienced LPN to join our team as a Nurse Case Manager . In this field-based role, you’ll make a meaningful difference by ensuring clients receive the highest quality of care in the comfort of their own homes.

What You’ll Do

  • Travel daily to visit current and new clients
  • Conduct quality assurance visits to monitor and evaluate care
  • Advocate for clients and facilitate communication with families
  • Conduct consultations with prospective clients and families
  • Provide training and ongoing support to Care Professionals
  • Identify opportunities to expand services within our client base
  • Supervise the QA Coordinator

    • Requirements

  • Licensed LPN in the state of Illinois
  • Ability to be in the field daily during working hours
  • Preferred experience in the senior care industry
  • Previous supervisory experience
  • Ability to work occasional nights and weekends
  • Valid Illinois driver’s license
  • Ability to drive a company car

  • $77,000 – $81,000 annually (based on experience)
