REGISTERED NURSE I Total number of positions needed : 1 1) How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm? yes 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Both 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Maybe 6) What region are you in? Home health or Hospice? Home Health 7) What’s creating the request? - work load 8) To which location will this person report for assignment, full address, Including the OU (Location Code) 30 Piedmont Dr Winder, Ga 30680 Please confirm above location is correct and list any other territories that are to be covered by the candidates : 9) How many miles per week will the consultant travel? 30 / 40 a day? Weekly? 30-40 per day’ 10) What are the weekly units of productivity expectations? 25 / 30? 30 points per week for RN 11) What EMR systems will the consultant be using : HomeCare HomeBase, Oasis, Outlook- must know all of these proficiently 12) Will there be any on call? If so, how much? Yes, 1 weekend a month. 13) Do you want the candidate to have home health experience? Yes, a must.
