Job Title : Registered Nurse (RN) Case Manager – Home Health Specialist

Company : Home Health Care, Inc.

Location : Southwest Minnesota Region

Schedule : Full-Time (Monday to Friday, 8 : 00 AM – 5 : 00 PM), 1 weekend / month, 1 on-call shift / week

Compensation : $85,000 – $130,000 annually + $8,000 in bonuses during first year of employment

Responsibilities :

  • Lead comprehensive nursing assessments and individualized care plans
  • Conduct OASIS visits and coordinate services across care settings
  • Collaborate with physicians and providers to maintain clinical continuity
  • Deliver education, emotional support, and transition planning for families
  • Ensure accurate documentation and regulatory compliance

    • Qualifications :

  • MN RN license in good standing
  • 3+ years of clinical RN experience; home health preferred
  • Strong documentation, care planning, and assessment skills
  • Reliable vehicle and valid driver’s license

    • What You’ll Do :

  • Perform assessments, develop care plans, and conduct skilled nursing visits
  • Serve as liaison between care providers, clients, and family members
  • Educate clients and caregivers on chronic disease management and safety
  • Ensure full compliance with OASIS and state / federal regulations

    • Qualifications :

  • Current MN RN license
  • Minimum 3 years of RN experience
  • Familiarity with home care documentation and regulations
  • Valid driver’s license and reliable vehicle
