Job Title : Physical Therapist
Job Description
Evaluate patients, determine treatment plans, and communicate with the care team. Prepare documentation such as evaluations, daily notes, progress notes, and discharge summaries. Consult with the rehabilitation team to select treatment programs and coordinate services. Develop home programs to enhance patient performance. Create treatment protocols using evidence-based guidelines and patient-specific goals. Initiate and respond to referrals from healthcare professionals. Maintain a positive service-oriented approach with patients, families, caregivers, and staff. Monitor patient progress and adjust treatment plans as needed. Train and evaluate therapists, and assist with orientation of new staff. Manage equipment maintenance and supply inventory. Commit to reducing costs and improving productivity. Handle changing patient schedules and maintain positive relationships with the healthcare team. Assist with Quality Assurance reports and facility reports. Participate in program development, department meetings, and comply with policies. Promote company philosophy and ensure quality care delivery. Respect patient privacy, confidentiality, and property. The pay range for this position is $95517.00 - $114620.00 / yr.
