West Virginia Licensed Care Manager RN

A company is looking for a Care Manager RN.

Key Responsibilities

Manage and triage member self-referrals to care management programs

Assist members in understanding their medical benefits and connecting them with in-network providers

Review and evaluate Health Risk Assessment data to drive the development of programs and services

Required Qualifications

Current unencumbered licensure as a Registered Nurse

Three years of healthcare clinical experience

Bachelor's Degree in Nursing or currently enrolled in a BSN program (completion within three years preferred)

Experience managing Medicare and / or Medicaid populations is preferred

Two years of Care Management experience is preferred
