A Skilled Nursing Facility in Camillus, NY is currently seeking a compassionate and dedicated Social Worker to join our team full-time.

Key Responsibilities :

Support residents in maintaining or improving their ability to meet daily physical, emotional, and psychosocial needs

\n

\n

Assess and identify each resident’s unique social and emotional needs

\n

\n

Create and update personalized care plans based on ongoing assessments

\n

\n

Collaborate on admission planning, discharge coordination, and transfers to other care settings

\n

\n

Participate in interdisciplinary care plan meetings to address residents’ social and emotional well-being

\n

\n \n

Qualifications : \n \n

LMSW preferred

\n

\n

Experience in long-term care or sub-acute settings required

\n
