Description : \n\nSUMMARY : \n\nUnder the direction of the Director of Nursing, the Outpatient Registered Nurse (RN) is responsible for delivering comprehensive, trauma-informed, and integrated medical care to clients receiving services through SFRC's Certified Community Behavioral Health Clinics (CCBHC). This includes conducting nursing assessments, performing point-of-care (POC) testing, supporting the management of chronic conditions, and coordinating care with behavioral health and primary care providers.\n\nAs a member of the interdisciplinary team, the RN works collaboratively to promote whole-person care and uphold CCBHC standards for quality, access, and accountability. The RN ensures compliance with all applicable state and federal regulations, nursing practice standards, and SFRC policies and procedures.\n\nREPORTING : \n\nReports to the Director of Nursing and Program Director\n\nDUTIES AND RESPONSIBILITIES : \n\n

  • Conduct nursing assessments and evaluate health needs of clients with co-occurring behavioral and physical health conditions.\n\n
  • Perform point-of-care testing (e.g., glucose, HbA1c, lipid panels, urinalysis, rapid infectious disease screens, drug screens) to support timely and evidence-based decision-making.\n\n
  • Monitor chronic medical conditions and provide follow-up interventions in collaboration with the interdisciplinary care team.\n\n
  • Coordinate medical care and referrals to primary care and specialty services, ensuring integration of behavioral and physical health care.\n\n
  • Collaborate with therapists, case managers, peer support specialists, and other team members to maintain a therapeutic environment and ensure continuity of care.\n\n
  • Participate in treatment planning and care coordination meetings, offering clinical insight to support whole-person care plans.\n\n
  • Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR) in accordance with CCBHC standards and regulatory requirements.\n\n
  • Provide health education and wellness counseling to clients and their families, promoting preventive care and improved health literacy.\n\n
  • Support quality improvement initiatives, including data collection for CCBHC quality metrics and outcome measures.\n\n
  • Remain informed of current practices in integrated care, behavioral health nursing, and trauma-informed approaches.\n\n
  • Attend mandatory staff meetings, training sessions, and continuing education to maintain licensure and stay current on best practices.\n\n
  • Maintain client confidentiality in accordance with HIPAA, 42 CFR Part 2, and SFRC's privacy and ethical policies.\n\n
  • Perform duties within the New Mexico Board of Nursing Scope of Practice for RNs.\n\n
  • Perform other related duties as assigned by the Director of Nursing or Program Director.\n\nPM21\n\nRequirements : \n\nSPECIAL SKILLS : \n\n
  • Strong assessment and clinical judgment skills with an emphasis on behavioral health and chronic disease management.\n\n
  • Proficiency with point-of-care testing procedures and interpretation of results.\n\n
  • Excellent written and verbal communication skills for documentation, interdisciplinary communication, and client engagement.\n\n
  • Technologically proficient, including use of Electronic Health Records and Microsoft Office Suite.\n\n
  • Ability to work effectively within a team-oriented, collaborative care model.\n\n
  • Composure and professionalism in a fast-paced outpatient environment.\n\n
  • Cultural responsiveness and commitment to serving the diverse populations of New Mexico.\n\nMINIMUM QUALIFICATIONS : \n
  • Registered Nurse License.? Must be licensed at the time of hire, and continue licensure throughout employment.\n
  • One-year experience working with clients with co-occurring mental health / substance abuse problems preferred.\n
  • Must be computer literate in order to effectively maintain compliance with licensing and certification standards and requirements for use of computerized client assessments, data systems, etc.\n
  • Must have and maintain a current, valid NM Driver's License.? Must also have and maintain a clean driving record, as well as auto insurance.\n
  • Bi-lingual / Bi-cultural preferred (English / Spanish).
