Job Description Our client, a Federally Qualified Health Center (FQHC) with multiple locations in New York City, is hiring a Family Nurse Practitioner . Play a vital role in providing comprehensive primary care services to patients, focusing on wellness visits, disease management, and prevention. Work collaboratively with a multi-disciplinary team to deliver high-quality medical care, addressing the complex needs of patients with comorbidities such as diabetes, hypertension, and HIV. Your expertise will be essential in delivering specialized medical care, including gender-affirming care and opioid addiction treatment. Responsibilities Provide primary care services as part of the primary care clinical team Conduct wellness visits, manage care, and prescribe / administer treatment Analyze records, reports, and test results to prevent and / or diagnose illness, disease, or injury Collaborate with other healthcare professionals to deliver medical services Manage labs, referrals, and other job-related duties as required Requirements : Current certification as a Family Nurse Practitioner (FNP) Master's degree in Nursing (MSN) with a focus on Family Nurse Practitioner 1-2 years of experience in primary care, preferably in a clinical setting Strong clinical skills, including assessment, diagnosis, and treatment Excellent communication and collaboration skills Ability to work in a fast-paced environment with multiple priorities Familiarity with ECU EMR system (training provided if necessary) Experience with population health management and care coordination, preferred Knowledge of LGBTQ health and wellness, preferred Bilingual or multilingual skills, preferred Some of the Benefits : Competitive salary range Productivity incentives Opportunities for professional growth and development Salary : The posted range is not a guarantee.
