Qualifications :

Must have MN and IA licenses

50% Estherville, IA SNF

50% Jackson, MN SNF and HH in that area

Drive Time and Mileage paid between facilities and during HH travel

Skilled / Rehab and Home Health - Post Acute or Long Term residents and Outpatients

Offering at least 13 weeks and option for longer term - 1 year

Flexibility in schedule for time off requested or to shift worked days : Normal is M-F could do Su-Th to have F off
