You will administer skilled nursing care to patients on an intermittent basis in their place of residence.

Brief overview of Responsibilities :

  • Assist the Registered Nurse in carrying out the plan of care for each patient
  • Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
  • Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
  • Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients and their families.
  • Help teach our patients and their families appropriate self-care techniques.
  • Active participation in team and staff conferences to ensure well-coordinated care for patients.
  • Prepare clinical and progress notes based on patient visits.
  • Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, as appropriate.
  • Participation in a weekend on call rotation
  • Other duties as assigned
  • Qualifications :

  • Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • At least one year of hospice experience required.
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available for use during work hours.
