Position Overview
The LTSS Service Care Manager provides field-based, member-facing care coordination and case management services to members enrolled in the Long-Term Services and Supports (LTSS) program. This role ensures that members receive comprehensive, person-centered care through assessments, follow-ups, and collaboration with interdisciplinary teams.
Key Responsibilities
- Conduct biannual member visits and document findings in accordance with program standards.
- Complete WLI / tasks as assigned and maintain compliance with our documentation requirements.
- Coordinate care with multi-disciplinary healthcare teams to support members’ individualized care plans.
- Perform follow-up calls with members to ensure ongoing support and service delivery.
- Participate in additional LTSS or special projects as assigned.
- Engage in daily check-ins with Team Lead and Manager; complete training and shadowing of other Service Coordinators prior to independent fieldwork.
Education & Licensure
Required :
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)Preferred :
