Facility requires 2 forms of government ID. One must be a photo ID. Required Required - Primary Source Verification Nursys verification required as applicable (required for all RNs) Complete Application / Resume A copy of the BSN / ASN diploma is required for all nursing staff. Alternating weekly 3x12 & 4x12 Nights 7p-7a Weekdays, EOW, Holidays within contract 3x12 one week and 4x12 the next - nights Alternating weekends Skills Required : FM Intermediate, IV Start, Triage, Scrub, Circulate Proof of I9 or Attestation Government issued photo ID required. Photo Identification must be carried on person during hours of service to client. Required. Facility requires 2 forms of government ID. One must be a photo ID. Please collect DL or Passport along with SSC. Required As required. ARC, AHA, or United States Department of Defense only. Required as Applicable by Profession. Must meet or exceed the Angoff score or otherwise achieve a passing score if not done through Relias.
