Position Responsibilities
Coordinates chart review / pre-visit planning efforts, ensuring all recent clinical documents are available at the time of visit, including : test results, specialist consults, discharge summaries, etc.
Collaborates with the Practice Manager to conduct Daily Huddles and Weekly Care Team meetings, in which the team will review practice population health data to improve care and achieve practice goals.
Provides oversight to patient flow by ensuring Medical Assistants efficiently perform the duties below :
Coordinates Population Health efforts :
Under the direction of the clinician, coordinates Transitions of Care :
Collaborates with the Practice Manager to coordinate clinical operations :
Required Experience
1-3 years of Medical Assistant experience in a medical office preferred.
Required Education
High School Diploma or Equivalent. Graduate of an accredited Medical Assistant program.
Training / Certification / Licensure
Must have current certification or registration as a Medical Assistant. BLS Certification Required.
Hourly Rate : $19.99 - $29.99 The actual salary / rate will vary based on applicant's experience as well as internal equity and alignment with market data.
