job_description.job_card.job_descriptionProvide direct client support in an integrated behavioral health program within a community mental health center Manage data entry, tracking, and scheduling to support a disease management and stepped care approach Obtain and record vital signs, summarize client history, and collaborate on follow-up care Assist in case management, connecting clients to wellness services and essential resources Administer medications, maintain Medication Administration Records, and ensure proper documentation Facilitate outreach to Federally Qualified Health Centers (FQHC) and other healthcare providers Verify client financial and demographic information for accurate records in the electronic health system Support the Behavioral Health Home care model, working alongside healthcare professionals Education : Bachelor’s Degree in Psychology, Social Work, Counseling, or related field preferred Experience : Minimum of two years in direct client care within a behavioral health, hospital, medical office, residential treatment center, homeless shelter, or community agency setting Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) certification required If CMA, certification must be from AAMA, NHA, AMT, or another accredited program (proof required)

Hours : Monday – Friday 8-5
