We are seeking a compassionate and skilled Occupational Therapist to work one-on-one with our patients in their home environment.

The selected candidate will be responsible for evaluating and assessing patient needs, formulating treatment plans that cater to the client's unique requirements.

• Evaluate and interpret patient evaluations and test results
• Determine occupational therapy treatment plans in consultation with healthcare professionals
• Administer occupational therapy treatments as ordered by a physician and oversee the delivery of services
• Evaluate effects of occupational therapy treatments and recommend adjustments and modifications
• Maintain equipment and workspace in a safe and clean condition
• Coordinate discharge planning and ensure continuity of care

To be successful in this role, applicants must possess a minimum of 2 years of occupational therapy experience, current licensure, and valid CPR certification.
