We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to join our team. In this role, you will provide skilled nursing care to patients in their homes, ensuring quality, continuity, and patient-centered care. The ideal candidate is self-motivated, organized, and comfortable working independently while maintaining a high standard of clinical excellence and patient communication.
As a Home Health RN, you will perform comprehensive assessments, develop and manage individualized care plans, and coordinate with interdisciplinary healthcare teams to promote positive outcomes. Youll provide education to patients and families, monitor progress, and ensure compliance with care goals and physician directives.

- Contract Type : Full-time, 13- week Contract
- Schedule : Monday Friday, 8 : 00 AM 5 : 00 PM
- Caseload Expectation : 2532 points per week (typically 35 patient visits per day)
- Setting : Field-based (patient homes)
- Electronic Documentation System : Home Care Home Base (HCHB)
- Assessment Tool : OASIS
- Travel : Daily travel to patient homes within assigned territory (mileage reimbursement provided)
Key Responsibilities :
Conduct comprehensive OASIS assessments and create individualized care plans.Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.Evaluate patient progress, identify changes in condition, and promptly communicate with the care team and physicians.Educate patients and families on disease processes, treatment plans, medications, and preventive care.Maintain accurate and timely documentation in Home Care Home Base (HCHB).Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care.Participate in case conferences, staff meetings, and continuing education as needed.Adhere to all regulatory and agency standards for documentation and patient care delivery.Job Requirements :
Active RN License in the state of practice (in good standing)Minimum of 1 year of recent Home Health experience (field experience preferred)Proficiency in Home Care Home Base (HCHB) requiredOASIS experience requiredStrong clinical assessment and critical thinking skillsReliable transportation, valid drivers license, and auto insuranceAbility to work independently and manage a flexible field-based scheduleExcellent communication, time management, and organizational skillsPreferred Qualifications :
