Salary :

This person will support our entire Austin service radius. All visits are in person at patient's home in various locations within the Austin 60 mile service radius. This is a PRN role where you will do scheduled visit that are scheduled a week or two out, doing a provider assessment, system assessment, and to see where our patient is at and what care is needed. This person will work approx. 3-5 hours (number of visits per week) and this person must be available during the traditional work week and hours. this person would largely travel throughout our Austin service radius.

Job Overview :

The primary function of the Nurse Practitioner role is to assist in overseeing the clinical care and treatment of our patients and their families. In addition, you will consult with the patients attending physician, in collaboration with our Medical Director(s). Also, you will provide clinical direction to the interdisciplinary team to assure the highest quality of care.

What Does The Nurse Practitioner Do Every Day?

• Collaborates with the IDT in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consult with and assist in the IDTs development of patient / family plan of care to assure quality care is provided.
• Educate the patient / family regarding disease process, self-care techniques, end-of-life care.
• Provide training to other clinical staff as needed.
• Perform and document visits to patients / family based on a plan of care.
• Interact with attending and consulting physicians as necessary regarding palliative care.
• Actively participate in the interdisciplinary team.
• Provide clinical assessment results to hospice physicians after making face-to-face encounters with patients.
• Provides on-call services to patients / families as assigned.
