Qualifications :
1. Current and unrestricted LPN license in Massachusetts
2. Current BLS CPR certification
3. Minimum of one year of nursing experience.

- Bilingual preferred (Spanish, Russian, or Vietnamese)
(Required to cover extended clinic hours, including evenings and weekends, on a rotating basis at any Caring Health Center location.)
Principal Responsibilities and Duties :
Provides coordinated care to adult and pediatric patients by collaborating with patients and interdisciplinary care team members to ensure comprehensive, patient-centered outcomes.Assists in conducting health assessments and screenings by recording medical histories, reviewing medications, observing and reporting patient conditions, and supporting individualized care plans under appropriate supervision.Assists providers with exams and procedures, ensuring patient safety, comfort, and appropriate documentation.Works effectively in both clinic and urgent care settings, utilizing critical thinking skills to support care prioritization and timely patient services.Performs nurse visits under supervision, providing assessments, patient education, and nursing interventions following established clinical protocols and scope of practice.Educates patients and families about health conditions, treatment plans, and preventive care, addressing gaps in care as directed.Provides team-based nursing care within assigned pods, collaborating with providers, RNs, and support staff to deliver coordinated, efficient care and ensure smooth transitions.Administers prescribed medications, vaccines, and treatments according to clinical protocols and documents accurately in the electronic medical record (EMR).Assists with patient triage via phone and in-basket messages, responding to requests within scope and escalating appropriately.Participates in health center outreach initiatives and community activities to promote public health and strengthen community engagement.Maintains strict confidentiality of all patient and health center information in accordance with HIPAA and organizational policies.Other duties as assigned.Working Conditions :
Work is performed in a busy outpatient clinic environment with direct patient contact.Requires frequent standing, walking, bending, and lifting.Involves the use of computers, phones, and other standard medical office equipment.Must be able to respond quickly in urgent or emergent situations.Requires flexibility to cover extended clinic hours, including evenings and weekends, on a rotating basis across multiple Caring Health Center locations.
