Colorado Mental Health Hospital in Pueblo (CMHHIP) — Social Work / Counselor III. Full Time. CMHHIP is a 24 hour behavioral health acute care facility that provides a safe, secure, and therapeutic setting for patients with both psychiatric and medical issues. The services include evaluation and treatment for patients who are severely mentally ill, with the goal of returning patients to their optimal level of functioning and assisting them in returning to the community as appropriate.
This work unit exists to provide inpatient psychiatric evaluations and treatment for severely emotionally disturbed and / or cognitively impaired older adults who cannot be safely or adequately treated in the community. Direct treatment utilizes individual, family, group work and psycho-educational treatment to increase mental health capacity and return patients to the community to live safe, productive lives.

Responsibilities
- Complete a comprehensive biopsychosocial assessment of patients being admitted for inpatient psychiatric treatment.
- Interview the patient to ensure reliable assessment data, engage the patient in a supportive and educative manner, maintain confidentiality, and evaluate symptoms and behaviors in a sensitive and safe manner.
- Contact / engage parents / family members, significant others, guardians, and referring agency personnel to interview and obtain collateral information to understand the patient's situation.
- Complete the Colorado Client Assessment Record (CCAR) and the bio-psychosocial component of the hospital's Integrated Initial Assessment document.
- Complete re-assessments of the patient's situation and response to treatment as frequently as daily and document progress in progress notes, weekly notes, and treatment plan reviews.
- Collaborate with and inform team members of relevant assessment information.
Discharge & Community Reintegration
Coordinate discharge planning through evaluation of patient needs relative to community resources and mobilize those resources.Serve as liaison between hospital and community agencies; identify and select appropriate community placements and treatment programs.Provide case management by making referrals to community agencies and maintaining reciprocal relationships with community resources (e.g., Social Security, Social Services, Court system, jails, DOC, probation, parole, DYC, attorneys, CMHC, CCB, nursing, boarding, and group homes).Attend meetings with the above-mentioned agencies.Treatment Planning
Participate in the direction and formulation of treatment plans and reviews.Collaborate with multidisciplinary treatment team to design interventions meeting patient and family education and therapeutic needs.Provide required documentation.Psychosocial Education
Assess patient and family education needs and identify readiness to learn, motivation, strengths, and barriers to learning.Provide ongoing psycho-social education relevant to individual, social, and environmental factors and medical regimens to support mental health growth and stability.Educate patients and families regarding symptoms, management, risk factors, relapse prevention, and level of care needs.Document written or typed notes of the above.Treatment
Conduct individual, group, and family therapy using social work theories and concepts appropriate to patient and family needs and backgrounds.Interpret treatment issues, expectations, and changes in behavior for patients and their families.Assess and document effectiveness of interventions toward goals.The Social Worker III guides milieu staff by modeling patient interventions and assigning practice or homework as appropriate.Professional Development
Participate in orientation, workshops, social work discipline meetings, and training to maintain competency.Provide consultation and training on social work services with clinical staff.Participate in clinical and administrative supervision to enhance skills, knowledge, and understanding of cases and processes.Minimum Qualifications
Current, valid Colorado or State participating in the Social Work Licensure Compact licensure as one of the following : Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW) OR Licensed Professional Counselor (LPC) OR Licensed Marriage and Family Therapist (LMFT) OR Licensed Addiction Counselor (LAC).
AND One (1) year of experience with current, valid licensure from a state participating in the Social Work Licensure Compact based on the work assigned to the position.
