Take charge of developing, implementing, and managing individualized patient care plans in a compassionate hospice and homecare setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes across all age groups, from newborns to geriatrics. Engage actively in delivering exceptional customer service and fostering respectful relationships with patients, families, and colleagues.
Essential Functions
- Create collaborative, personalized care plans based on medical and nursing diagnoses and patient goals.
- Provide skilled nursing services aligned with the care plan.
- Complete accurate OASIS assessments as required.
- Coordinate with physicians, patients, families, and other disciplines to adjust care plans as needed.
- Supervise Home Health Aide services according to policy.
- Participate in on-call rotations to ensure 24 / 7 nursing availability for hospice, homecare, and private duty patients, including scheduled visits and admissions.
- Maintain timely and precise documentation following agency guidelines.
- Build and sustain positive working relationships with patients, families, healthcare providers, and referral sources.
- Promote patient independence by setting care goals and involving patients and families in discussions about medical issues, testing, medications, and self-care skills.
- Observe and assess patients' physical, mental, emotional, and spiritual conditions, collaborating with team members as appropriate.
- Adhere strictly to HIPAA regulations to protect patient health information.
Minimum Knowledge and Skills

Graduate of an accredited nursing program.Several years of clinical nursing experience.Preferred : Bachelor of Science in Nursing (BSN).Preferred : Previous hospice and / or home health experience.Preferred : Certification as a Hospice and Palliative Care Nurse.Licenses and Certifications
