Nationwide Therapy Group is seeking a travel nurse RN Long Term Care (LTC) Case Management for a travel nursing job in Lakota, North Dakota.

Position : Registered Nurse – Case Manager

Shift : Day Shift – AllH Days (exact hours TBD)

Guaranteed Hours : 40 hours / week

Contract Length : 15 weeks (likely renewable or long-term placement)

Job Description & Responsibilities

As an RN Case Manager in a post-acute setting, you will :

  • Coordinate discharge planning care transitions , and interdisciplinary communication
  • Advocate for patient outcomes and continuity of care
  • Work the floor as needed to assist with patient care if staff coverage is low
  • Collaborate with providers, social workers, families, and therapy teams
  • Chart accurately in PointClickCare
  • This is a hybrid role requiring both clinical bedside support and case management skills

    Special Notes

  • Floating to the floor is required when staffing gaps arise
  • 2-week cancellation policy
  • Must be comfortable in both clinical and administrative roles
