About The Position

Job Title : Registered Nurse

Hours per Shift : 8 hours (08 : 00 - 17 : 00)

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on specialty.

License : Must have a valid nursing license

Certifications : CPR, BLS

Must-Have :

• Proficient in HomeCare HomeBase (HCHB) and OASIS, with experience using both within the last 6 months.
• Reliable transportation and current driver's license.
• Agency-required liability insurance.
• Willingness to travel up to 75 miles for home visits.

Description :

Provides and coordinates home nursing care according to physician's orders.

Responsible for applying the nursing process competently based on agency policies and procedures.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Must attend periodic office meetings.

Responsible for home visits to patients and potentially visiting care facilities.

Must accept every patient scheduled regardless of distance.

All documentation must be completed within 24 hours of the appointment.

No guaranteed hours; must meet weekly productivity points of 30.

Opportunity to float to another location for more hours if patient census is low.

Weekend and weekday on-call rotation may be required.

Must be open to traveling longer distances between patients.

All travelers must be proficient in required EMR systems.
