• The scope of this position requires overall coordination of an assigned group of clients and caregivers.
• Ensure that client schedules are complete, caregiver schedules are optimized, and all are always receiving the highest level of services.
• Frequently communicates with clients, their families, case managers and AHC staff members to ensure continuity and coordination of client care services.
• Responsibilities and qualifications of this position are to meet and comply with Homecare policies and procedures, federal and state rules and regulations, and to adhere to confidentiality guidelines as set forth in the Health Insurance Portability and Accountability Act of 1996.
