About The Position

Job Title : Occupational Therapist

Shift : Monday to Friday, Days

Hours per Shift : 32 hours guaranteed

Experience : New graduates are welcome; SNF experience preferred

License : Valid state license required

Certifications : License verification needed

Must-Have : Completed prescreen form, resume, and license verification must be submitted

Description :

Ensure that the prescreen form is filled out and attached upon submission.

A resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

Candidates should be prepared to undergo a COVID test 72 hours prior to the start date, with results issued 48 hours prior.

Please review the credentialing checklist prior to submission.

This includes state license verification, resume, three references, and background checks.

Background checks include a county criminal search, federal criminal search, multi-state criminal search, nationwide sex offender search, HHS / OIG search, GSA search, and social security number trace.

Candidates must also have a 2-step PPD or Quantiferon test results or proof of a positive PPD and chest X-ray.

Candidates should be aware of the position's urgency.
