Job Title : PRN MSW Home Health Social Worker

Location : Venice, North Port

POSITION SUMMARY :

  • Assess patient's emotional, environmental, social and financial needs, and develop appropriate care plans.
  • Coordinate patient care with other healthcare professionals, such as physicians, nurses and therapists.
  • Monitor and evaluate patients' progress, and adjust care plans as necessary.
  • Educate patients and caregivers about resources and services available in the community.
  • Provide counseling and emotional support to patients and families.
  • Maintain thorough documentation of patient care in electronic medical records, as well as paper files.

    • Qualifications :

  • Master's degree in Social Work from a school accredited by the Council on Social Work Education.
  • Active and valid state license as a Social Worker.
  • Minimum of one year of experience in the field of healthcare, home health or hospice preferred.
  • Knowledge of community resources, and experience in navigating and coordinating care with local agencies.
  • Excellent communication skills, both verbal and written.
  • Ability to work independently, and to manage and prioritize multiple tasks in a fast-paced environment.
  • Must have a reliable vehicle, valid driver's license, and auto insurance.
