Job Summary :
This is an 18-week contract position for a Licensed Clinical Social Worker (LCSW) experienced in Hospital Care Management and discharge planning. The LCSW will be responsible for providing social work services, care management, and discharge planning within a hospital setting. The contract is expected to continue into January, with the earliest end date being March 30.
Location : Newberg, Oregon, United States
Responsibilities :
- Provide care management services.
- Perform discharge planning.
- Collaborate with the healthcare team.
- Perform data entry into electronic medical database systems.
- Document patient care and related duties appropriately.
- Perform selected patient registration activities.
- Coordinates equipment / supply needs. Notifies appropriate department of equipment malfunction.
- Perform any other related duties (e.g., clerical, technical duties) as assigned.
