Job Summary :
The Licensed Practical Nurse (LPN) will provide nursing care to geriatric long-term care residents, focusing on medication administration and oversight of CNA colleagues. The LPN will work evenings / nights, weekends, and holidays as per unit needs, providing direct patient care, including wound care, in a long-term care environment. The role requires critical thinking skills and the ability to work independently while providing care with dignity and respect.

Responsibilities :
- Provide direct patient care to geriatric long-term care residents.
- Administer medications and treatments as prescribed.
- Oversee and guide CNA colleagues.
- Perform wound care.
- Monitor and document patient conditions, reporting changes to the appropriate healthcare professionals.
- Utilize critical thinking skills to assess and address patient needs.
- Work independently while maintaining a high standard of care.
- Ensure patient dignity and respect in all interactions.
- Gather and report patient data.
- Carry out orders and perform therapeutic procedures.
- Document patient care and related duties appropriately.
Required Skills & Certifications :
Active NYS LPN license.BLS certification from the American Heart Association.Minimum of 2 years of experience in a long-term care setting.Preferred Skills & Certifications :
EPIC experience in the same area / specialty within the last year.Special Considerations :
Evenings / Nights, weekends and holidays per unit needs.Must be able to obtain a TB skin test.Scheduling :
