- Conduct thorough nursing assessments using appropriate standards, recognizing changes in patient conditions, and validating priorities with the healthcare team.\n
- Collaborate with patients, families, and the healthcare team to create tailored care plans addressing the unique needs of each patient.\n
- Continuously update care plans based on patient condition and team communications.\n
- Set realistic and measurable goals for patient discharge, ensuring alignment with patients and families.\n
- Respect and incorporate patients' cultural, spiritual, and personal beliefs into care planning.\n
- Facilitate a smooth transition of care post-discharge with necessary referrals.\n
- Provide direct and efficient nursing care as per the established care plan.\n

- Utilize performance indicators to evaluate how effectively care is being delivered and identify opportunities for improvement.\n
- Assess patient and family learning needs and provide educational support based on their understanding.\n
- Act as a patient advocate, coordinating care with healthcare professionals.\n
- Maintain professional standards, continuously evaluating personal nursing practice and seeking opportunities for education and improvement.\n
- Document patient care meticulously to support overall healthcare needs.\n

- This is a non-management position reporting to a supervisor, manager, director, or executive.\n\nMinimum Requirements\n
- Education - Associate's degree in Nursing; Bachelor's degree preferred.\n
- Experience - Minimum of one (1) year as a registered nurse.\n\nIn Lieu Of\n
- We will consider successful completion of Prisma Health Nurse Residency Program or a comparable residency from another organization in place of one (1) year of registered nurse experience.\n
- One year of LPN experience within the same service line will be accepted for internal candidates.\n
- A nursing diploma will be accepted instead of an Associate's degree, provided licensure to practice as a Registered Nurse is held.\n\nRequired Certifications, Registrations, Licenses\n
