About The Position

Job Title : Registered Nurse

Hours per Shift : 8 hours (08 : 00 - 17 : 00)

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty.

License : Must have a valid nursing license in hand.

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase EMR within the last 6 months.

Proficiency in OASIS within the last 6 months, or completion of a retraining course during compliance.

Must be willing to travel up to 75 miles from the assigned care center.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician orders.

Care is based on agency policies and procedures through the competent application of the nursing process.

Responsible for the oversight of Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Daily patient visits will occur in home settings, with some visits to care facilities.

The average number of patients seen per day is between 5-10, equating to approximately 6-8 points in productivity metrics.

The contractor will not have guaranteed hours; ongoing communication with management is necessary to discuss needs.

All documentation must be completed within 24 hours of each patient appointment.

Delayed or incomplete documentation that impacts billing and follow-up patient care may lead to cancellation of the assignment.

On-call shifts may be required, rotating every 4th weekend.

The candidate must be comfortable accepting all assigned patients regardless of travel distance.
