Applications shall be picked up in the Mayor's Office at 126 West 6th Street, East Liverpool, Ohio 43920 or requested at beginning Friday, October 17, 2025.

Applications must be returned to the Mayor's Office, 126 West 6th Street, East Liverpool, Ohio 43920 in person or by E-mail with required attachments by 4 : 00 P.M., Thursday, October 30, 2025.
