Shift Details : FT – 40 hours / week

Schedule : Monday-Friday – 8 : 00 AM-4 : 00 PM, with some flexibility

Compensation : $49,000-$54,500 annual salary, based on experience. Please feel free to apply if you are close to meeting the minimum requirements, this may impact pay range.

About the job :

VOA is hiring a Mental Health Case Manager to provide coordination of services to support adults with mental illness and their families in Counties around the Twin Cities Metro area. Case Managers help coordinate services to support areas such as mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation.

Essentials :

Maintain own caseload of 24 clients and assist with case coverage when needed. These duties include :

Provide services in the four core case management components-Assessment, Service Plan Development, Referral and Linkage and Monitoring, and Coordination of Services.

Meet with the client as described in the client’s individualized plan and program policies.

Keep necessary case notes documenting contact with the client and collateral contacts.

Develop and utilize available area resources and make appropriate referrals.

Develop and update the client's individualized plan. Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.

Coordinate with a mental health professional to assure the completion of a mental health assessment of client needs and to develop individualized plans.

Maintain client case files, including completing required client documentation in a timely fashion as defined in program policies.

Complete monthly billing documentation

Coordinate services with the client and other members of the multi-disciplinary team.

Assist clients in connecting with community resources.

Maintain a work environment which respects and supports cultural responsivity, upholds VOA's values, and promotes staff engagement and health morale.

Meet the requirements for clinical supervision by attending individual and group supervision, case consultation and reviews, and staff meetings.

Attend ongoing staff development opportunities on pertinent topics to meet training requirements.

Some regularly scheduled onsite / in-person attendance expected at our New Hope office.

Travel to settings where services are provided, primarily family, home, or school.

Required Qualifications :

BA / BS in behavioral sciences or social service field

2,000 hours of supervised experience in the delivering mental health services to adults with a serious and persistent mental illness is preferred.

Must have a valid driver's license
