Visiting Nurse Service and Hospice of Suffolk (VNSHS) is an independent not-for-profit healthcare agency that serves patients and their families throughout Suffolk County Long Island with visiting nurse home and hospice services, inpatient Hospice House, rehabilitative therapy, medical social work, home health aide care, nutritional education and diabetic counseling, community programs, and more.

Some Job Responsibilities :
Supervises, instructs, guides, and supports nursing and auxiliary personnel to provide high quality services to patients and their families consistent with the hospice philosophy and agency mission.
Assumes direct report responsibilities for all Hospice House and Hospice field staff.
Assumes responsibility to cover duties of Hospice Director of Patient Services in the absence of the Hospice DPS.
Conducts periodic performance evaluations of all direct reports through activities that include but are not limited to observation of clinician / patient interaction, care plan review, documentation by clinician, and case review in conference or within IDG team meetings. Identifies trends that need to be addressed by clinician.
Oversees staff scheduling to ensure adequate staffing 24 / 7 at the Hospice House.
Participates in orientation of new hires and serves as role model demonstrating by example.
Leads interdisciplinary group (IDG) meetings, ensuring they are conducted in accordance with regulations and fostering a collaborative environment.
Interprets regulatory guidelines as apply to provision of the Medicare Hospice Benefit.
Performs duties related to drug destruction in the Hospice House.

Salary :
$115,000.00 - $135,000.00 per year

Job Qualifications :
Registered Nurse license to practice in New York State.
Baccalaureate degree in Nursing or related field preferred.
Two years’ experience in the delivery of Hospice Care.
Demonstrated leadership ability.
Competent computer skills.
