Job Summary :

The ACT Registered Nurse (RN) is an integral member of the ACT multidisciplinary team and is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals and collaborating with community providers, providing management and administration of medication in conjunction with the psychiatrist, delivering a range of treatment, rehabilitation, and support services, and sharing case management responsibilities with other staff. The Registered nurse provides chart management for an assigned group of clients, including responsibility for developing, writing, and revising overall treatment goals and plans in collaboration with other team members. This position also includes educating and supporting clients’ families and advocating for clients’ rights and preferences.

Duties / Responsibilities :

The Registered Nurse provides nursing care and health education to individuals served by the ACT team, with the majority of services delivered in the community (e.g., individual’s home, hospitals, shelters, etc).

Services include :

Manage the medication system, administer and document medication treatment.

  • Transcribe and fill prescription orders.
  • Oversee and monitor medication storage and inventory.
  • Assemble medication deliveries, manage IM injections, and maintain accurate documentation (e.g.,MAR).
  • Communicate medication changes with the team in real-time.
  • Screen and monitor individuals for medical problems / side effects.

  • Conduct regular screenings and assessments for medical conditions and medication side effects.
  • Monitor and response to pre-existing medical conditions.
  • Communicate and coordinate services with other medical providers .

  • Document services in the Electronic Medical Record.
  • Participate in daily team meetings and treatment planning reviews.
  • Attend medical appointments with clients when possible.
  • Maintain communication with inpatient / outpatient providers using health communication forms.
  • Engage in health promotion, prevention, and education activities.

  • Provide symptom education to improve self-awareness and self-management.
  • Assess and support behavior change around risky behaviors.
  • Offer education and support around chronic illness, preventative screenings, and wellness.
  • Promote smoking cessation and health self-management strategies.
  • Educate team members about psychiatric symptoms and side effects.

  • Provide formal and informal training to team members regarding medication side effects and symptom management.
  • Contribute to team-wide understanding during daily meetings.
  • Support medication adherence strategies.

  • Collaborate with the psychiatrist and team to improve adherence using tools such as behavioral tailoring, pill organizers and reminders, long-acting injectable medication when appropriate, etc.
  • Miscellaneous :

  • Participate in rotating on-call responsibilities.
  • Provide psychiatric rehabilitation and ADL support in the community, including, but not limited to maintaining housing, supporting personal hygiene, nutrition, and household management, assisting with accessing entitlements or employment, coordinating primary and specialty medical care, and completing miscellaneous duties and projects based on client needs.
  • Maintain accurate and timely documentation of clinical records in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor.
  • Meet performance expectations for time and billing standards.
  • Participate fully as a member of the Spero Assertive Community Treatment Team as a supportive, reliable, positive, collaborative, and professional team member during the development of individual treatment planning, daily team meetings, and crisis intervention.
  • Participate in continuing education and professional development activities to maintain certification and stay current with best practices in peer support.
  • Required Knowledge, Skills, and Abilities :

  • Understanding of approaches and interventions used to support others in recovery and resiliency, demonstrating efforts as self-directed recovery.
  • Knowledgeable of community resources and how to connect individuals to those resources.
  • Ability to understand and carry out directions and instructions; ability to maintain confidentiality; interest and skill in providing services to individuals and groups; ability to speak, read, and write English.
  • Ability to provide documentation consistent with department standards for quality and timeliness; willingness to work hours conducive to reaching the identified population.
  • Ability to establish and maintain cooperative working relationships with individuals, coworkers, and other agencies.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid Minnesota Driver’s license and ability to transport clients.
  • Ability and willingness to discuss recovering experiences during the interview process.
  • Education and Experience :

    The Registered Nurse may have a master’s, bachelor’s, or associate degree in nursing, must have an unencumbered license to practice as a registered nurse in the state of Minnesota and should have at least one year of experience in an inpatient or outpatient psychiatric setting.

    Physical Requirements :

    Operates a motor vehicle; sees using depth perception, stands; sits; moves about the work area; clients 1 floor of stairs; lifts, pushes, pulls, moves carts and carries items weight up to 5 lbs.; operates a keyboard; speaks with a clear and audible voice; reads a 12pt. font; hears a normal speech level; works in clients homes or other areas exposed to dust or second-hand smoke; may work in uneven areas.

    Work Environment :

    This position operates within a dynamic and collaborative Assertive Community Treatment (ACT) team. You will work collaborate with a multidisciplinary team of professionals, including case managers, peer specialists, nurses, therapists, and psychiatrists, in a supportive and respectful environment. The ACT team provides services primarily in the community, including clients’ homes, community settings, and the office. This role requires flexibility and ability to adapt to changing situations. The team emphasizes a client-centered, recovery-oriented approach, valuing the unique experiences of each individual served.
