Responsibilities :

• Conducts a comprehensive assessment, including physical, emotional, and mental status for clients of all ages- infant through adult
• Develops plans of care with realistic outcomes, based on interpretation of assessment of data and consideration of client's preferences
• Performs designated discipline interventions according to agency policies
• Provides client / SO education on prevention or management of illness, functional disability, related changes in health status, and on medications and treatments
• Evaluates the results of clinical interventions to measure progress toward goals
• Reports all pertinent information about client status and progress to appropriate multidisciplinary health team members
• Plans sequence of home visits with attention to issues of acuity, geographical location, and other pertinent considerations

Requirements :

Hours : M-F 8 : 30 a.m. – 5 : 00 p.m. - some weekends and some holidays.

Salary : Starting annual base - $111,103.97
