Responsibilities : Serve as a primary care provider for your own panel of patients Deliver whole-person care addressing acute, chronic, and preventive needs Perform in-office women’s health procedures (e.g., pap smears, IUD insertion) Occasionally provide care in street-based / mobile settings Participate in a shared on-call rotation (approx. 3–5 calls / week every other week) Collaborate with a trauma-informed, harm-reduction care team Support Medication-Assisted Treatment (MAT) programs (training provided) ?? Qualifications : Active (or eligible) California NP license DEA license & BLS certification (or willing to obtain) Passion for working with underserved populations Open to MAT training and integrated care models Empathy, flexibility, and a team-first approach Spanish-speaking ability (preferred but not required) Experience working with LGBTQ+ communities or individuals experiencing homelessness Knowledge of FQHC practices and Electronic Medical Records Interest in long-term community-based practice and advocacy ?? As an FNP, you’ll be a key part of an interdisciplinary team working to meet the physical, emotional, and social health needs of individuals experiencing homelessness or living in deep poverty. This position combines traditional outpatient care with occasional mobile / outreach-based services.
