You will coordinate care with an interdisciplinary team, patients, families, and referring agencies, taking ownership of the care coordination process.\n\nQualifications : \n
- Graduated from an accredited nursing program (diploma, Associate, or Baccalaureate).\n
- 1-2 years of nursing experience preferred, within the last 3 years, in a psychiatric setting (such as acute treatment units, home care, or outpatient services).\n
- Current Registered Nurse license for the state.\n
- Valid state Driver's License.\n
- Proof of current CPR certification and a Hepatitis consent / declination form.\n
