You will coordinate care with an interdisciplinary team, patients, families, and referring agencies, taking ownership of the care coordination process.\n\nQualifications : \n
• Graduated from an accredited nursing program (diploma, Associate, or Baccalaureate).\n
• 1-2 years of nursing experience preferred, within the last 3 years, in a psychiatric setting (such as acute treatment units, home care, or outpatient services).\n
• Current Registered Nurse license for the state.\n
• Valid state Driver's License.\n
• Proof of current CPR certification and a Hepatitis consent / declination form.\n
