About The Position

We are seeking an experienced Speech-Language Pathologist (CCC-SLP) for a day shift, 5x8 schedule (08 : 00 – 17 : 00) in a skilled home care setting. The role involves managing dysphagia, conducting patient and family education, and providing skilled visits across multiple territories.

Job Requirements

Active state license and CCC-SLP certification

BLS certification

Minimum 1 year of home health experience (OASIS preferred)

Proficiency in evaluation tools (BIMS, oral peripheral exams, depression screening)

Ability to conduct 6–8 home skilled visits per day

Valid driver’s license, reliable car, and willingness to travel

COVID vaccination (or approved exemption) and flu shot / declination with mask compliance

Weekend availability (1–2 per contract)
