Jackson Ridge and Maquoketa Care Centers are seeking a compassionate and dedicated Licensed Social Worker to join our care team. This role supports the emotional and psychosocial well-being of our residents by providing individual counseling , facilitating group therapy sessions , and offering family counseling services .

Responsibilities :

  • Conduct one-on-one therapeutic sessions with residents.
  • Collaborate with interdisciplinary staff to support residents’ overall care plans.
  • Provide counseling and support to families navigating long-term care decisions.
  • Develop and lead multiple group therapy programs tailored to residents’ needs.
  • Collaborate with our Nursing Team and Physician to maintain best outcomes.
  • Proficiently chart and document sessions and services provided.

Requirements / Qualifications :

  • Active Iowa licensure as a Social Worker (LSW or higher).
  • Experience working in nursing homes or long-term care.
  • Experience working with patients with dementia induced behaviors.
