Position Summary
We are seeking a Certified Nurse Practitioner Supervisor to oversee and lead clinical operations and coordinate patient care within our PACE program. The NPS collaborates with Community Primary Care Physicians (CPCPs) and the Interdisciplinary Team (IDT) to ensure comprehensive, person-centered care that promotes the health, independence, and well-being of frail and elderly participants.
Key Responsibilities
- Coordinate and oversee participant care in collaboration with physicians and the interdisciplinary team.
- Conduct annual and as-needed wellness assessments, ensuring accurate documentation of diagnoses and comorbidities.
- Provide urgent care services, assess acute changes in condition, and perform timely post-hospitalization evaluations.
- Supervise assigned Nurse Practitioners and support the professional development of clinical staff.
- Serve as Acting Medical Director in the absence or at the direction of the Medical Director.
- Oversee the development and implementation of individualized care plans, ensuring quality, compliance, and positive outcomes.
- Participate in complex care management, interdisciplinary team meetings, and quality improvement initiatives.
- Conduct home visits and provide emergency coverage for PACE center providers as needed.
- Ensure compliance with HIPAA, regulatory requirements, and documentation standards.
Qualifications
NY State Nurse Practitioner LicenseBoard Certification / Eligibility : AANP or ANCCSpecialties : Geriatrics, Primary Care, or Internal MedicineDEA CertifiedTwo (2) years in clinical nursing practice with in-home care or care managementOne (1) year working with elderly / frail population or open to receiveProficient in Microsoft Office and EHR systems
