About the Job :

Become part of the team at Oregon Provider, a locally owned, family-run Medicaid agency dedicated to empowering individuals with developmental and intellectual disabilities. Job Site Locations :

We proudly support clients across Clackamas, Multnomah, and Washington Counties in Oregon. We match staff with assignments near their home to reduce commute time and support a healthy work-life balance. Our scheduling approach is designed to avoid overtime and promote consistency for both staff and clients.

Position Details :

  • Full-Time : 40 hours per week.
  • Part-Time : 20 hours per week.
  • Direct Support Professionals $23.00 Per Hour.
  • Vocational Rehabilitation Employment : $25 Per Hour.
  • ODDS Job Coaching : $25 Per Hour.
  • Discovery & OCB Specialist $30.00.

  • Paid Time Off (PTO) :

    • Accrue up to 40 hours annually starting after 90 days.

  • Increase to 80 hours annually beginning your 5th year.
  • Paid travel time between client shifts.
  • Ongoing mentorship, training, and opportunities for professional growth.

    • Key Responsibilities :

  • Provide personalized in-home support with daily living tasks, personal care, and household management.
  • Deliver life skills training aligned with individual client goals, fostering greater independence.
  • Encourage and support communication skills, social interaction, and relationship building.
  • Assist with medication reminders and administration in accordance with care plans.
  • Schedule, coordinate, and accompany clients to medical, dental, and therapeutic appointments.
  • Offer transportation, shopping assistance, and advocacy to support client needs and preferences.
  • Promote community inclusion by engaging clients in local events, recreational activities, and volunteer opportunities.
  • Support clients in navigating community resources, employment, and educational programs.
  • Ensure safety, comfort, and dignity in all settings, whether at home or in the community.

    • Qualifications :

  • Successfully pass criminal, driving, drug, and Orchards, the state background check each year.
  • Demonstrate reliability, initiative, and accountability in all aspects of the role.
  • Maintain access to a smartphone or tablet for documentation and communication.
  • Be computer literate and able to complete required documentation and online training.
  • Complete a minimum of 24 hours of approved training annually.
  • Hold valid in-person CPR / First Aid and Food Handler’s certifications prior to hire.
  • Provide a dependable vehicle with valid insurance that meets 100 / 300 / 100 liability coverage requirements, with the Agency listed as an interested party for any changes to coverage during employment.
  • Exhibit compassion, patience, and professionalism when supporting clients and families.
  • Apply strong problem-solving skills and practical life abilities in everyday situations.
  • Confidently deliver hands-on support, training, and guidance to clients with diverse needs.
