Registered Nurse (RN) - Hospice Care\n\nSchedule : Monday-Friday, 8 : 00 am - 4 : 30 pm\nShift Type : Days (No weekends, No on-call required)\n\nOverview\n\nWe are seeking dedicated and compassionate Registered Nurses (RNs) to join our hospice care team. In this important role, you will provide high-quality clinical care to patients receiving end-of-life services, ensuring dignity, comfort, and exceptional support for both patients and their families.\n\nAs a candidate, you should be comfortable working independently, managing a daily caseload, and delivering thorough nursing assessments and interventions across various care settings.\n\nResponsibilities\n
- Conduct detailed nursing assessments for admissions, recertifications, routine, PRN, and death visits.\n
- Develop, implement, and assess individualized care plans in collaboration with the interdisciplinary team.\n
- Ensure quality of care and compliance through accurate documentation in the EMR system; experience with HomeCare Homebase is preferred.\n
- Maintain productivity benchmarks of 5-7 visits per day while delivering compassionate, patient-centered care.\n
- Collaborate with physicians, social workers, and other healthcare professionals to facilitate care transitions and service levels.\n
- Identify and address social determinants of health impacting patient care, such as housing and transportation.\n
- Provide health education, preventive care, and emotional support for patients and their families.\n
- Advocate for patient rights, ensuring confidentiality and ethical standards are upheld.\n
- Participate in interdisciplinary team meetings and contribute to initiatives for continuous quality improvement.\n\nQualifications\n
- Active RN license in good standing (state-specific).\n
- Current BLS / CPR certification is required.\n
- Valid driver's license and auto insurance.\n
- Previous hospice experience is necessary; proficiency in HomeCare Homebase is strongly preferred.\n
- Associate's or Bachelor's degree in Nursing (BSN preferred).\n
- Strong clinical judgment and critical-thinking skills in complex situations.\n
- Exceptional communication and organizational skills.\n
- Case Management certification is preferred but not required.\n\nKey Competencies\n
