• Resume and license verification attached
• Job Description :
• Complete and attach prescreen upon submitting a candidate; candidates will be declined if this is not completed
• Provide a detailed resume and state license verification
• Minimum of three references required
• County criminal background check for the past 7 years for all counties lived and worked in
• Federal criminal background check for the past 7 years or more
• Multi-state criminal search for the past 7 years or more
• Nationwide sex offender search for the past 7 years or more
• HHS / OIG search required
• GSA search necessary
• Social security number trace required
• SAM search required
• CORI Massachusetts background check
• Complete 8 hours of dementia training

Position urgency is classified as normal, bilingual candidates and holiday coverage are not required.
