job_description.job_card.job_descriptionMonday- Friday 8am-4 : 30pm Starting Rate : $40.00 / hour (+) based on years of experience Must possess a Bachelors of Science in Nursing. Current licensure as a Registered Nurse, through the Pennsylvania Board of Nurse Examiners A valid US state drivers license and a minimum age of 21 years old. Must be capable of attaining approved Melmark driver status. A minimum of one (1) year experience in a related field. Prior work experience with aging adults and adults with functional impairment. Must be a Certified School Nurse or willing to complete the certification within 1 year of hire. Case management includes interfacing with outside agencies, attending client meetings including IPP’s, ISP’s, IEPs and client reviews, assisting with the admission and discharge of clients, maintain updated lifetime medical histories, outreach to families and agencies as needed for coordination of care during adult day / school hours of programming administering medications as needed including tube medications through g-tube and j-tube ports, completing nursing assessments for illness or other medical issues, clinical and programmatic goals. Maintains a knowledge of medical insurance and reimbursement mechanisms as related to client pharmacy, physician and durable medical equipment needs, administration of treatments and oversight of feeding bags and pumps and the ability to intervene quickly in case of an emergency. Works closely with Primary and Staff nurses on site to provide comprehensive care. Responds immediately to Codes and medical emergencies with appropriate equipment. Supervises Medication Certified staff assisting with administration of medication and related responsibilities in 2380 programs Monitors the health and well-being of adult day clients and Melmark students. Maintains updated medication administration records and protocols as needed for adult day clients and students at Melmark School. Maintains case management responsibilities for those assigned to care. Communicates relevant healthcare concerns to physicians, appropriate healthcare staff, families, and assists in coordinating treatment process. In addition, communicates pertinent information to members of the interdisciplinary and healthcare teams. Attends and coordinates client medical updates with the interdisciplinary team meetings and reviews in a timely manner. Provides information to the Department of Quality Assurance pursuant to Melmark’s incident management policy. Primary responder to work related incidents / accidents of employees. Provides evaluation, treatment, makes appropriate referrals for treatment in accordance with established policies and procedures, and ensures that the proper documentation is completed. Completes all nursing documentation as required. Reviews all nursing documentation for accuracy and makes updates to the daily nursing report. School Nurse Overview
The Adult Day / School Nurse is responsible for assessing, implementing and evaluating the delivery of health care and health maintenance for clients served in the Melmark Adult Day program and the Melmark School. The Adult Day / School Nurse works closely with the Adult Day Program Directors, Education Administrators, Program Room Assistants, Teachers, ABACs, Residential Staff and other Melmark staff for comprehensive health care delivery during day program and school hours.
