Chapters Living, a comprehensive skilled nursing, independent living, assisted living, and memory care community, is seeking an experienced and compassionate Manager of Social Services. The ideal candidate will oversee the social services department, ensuring that residents' psychosocial needs are met in compliance with federal and state regulations.
- Plan, develop, organize, and implement the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations.
- Conduct comprehensive psychosocial assessments for residents upon admission, quarterly, and as needed, to identify and address psychosocial, mental, and emotional needs.
- Develop and implement individualized care plans based on assessment findings, collaborating with interdisciplinary teams to ensure a holistic approach to resident care.
- Provide crisis intervention and support to residents and their families, addressing issues such as adjustment to placement, grief, and loss.
- Facilitate discharge planning, coordinating with residents, families, and external agencies to ensure a smooth transition to other care settings or home.
- Maintain accurate and timely documentation in compliance with federal and state regulations, including progress notes, care plans, and assessments.
- Supervise and provide guidance to social services staff, including hiring, training, performance evaluations, and professional development.
- Stay informed about changes in federal and state regulations affecting social services and ensure departmental compliance.
- Participate in interdisciplinary team meetings, quality assurance programs, and other committees as required.
Requirements :
Bachelor's or Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).Current licensure as a social worker in the state of practice :Iowa : Licensed Bachelor Social Worker (LBSW), Licensed Master Social Worker (LMSW), or Licensed Independent Social Worker (LISW).Minimum of 3 years of experience in a healthcare or long-term care setting is required.Thorough understanding of federal and state regulations related to social services in long-term care facilities.Excellent interpersonal and communication skills, with the ability to work effectively with residents, families, staff, and external agencies.Strong organizational and time management skills, with attention to detail and the ability to handle multiple priorities.Proficiency in electronic medical records (EMR) systems and standard office software.State-Specific Requirements :

Adherence to Iowa Administrative Code Chapter 280 – Licensure of Social Workers, which outlines the scope of practice and licensure requirements for social workers.Completion of 27 hours of continuing education every two years, as mandated by the Iowa Board of Social Work.
